This website offers young people between the ages of 15 and 24 years old who live in either Staffordshire, Shropshire, Telford & Wrekin and Herefordshire the opportunity to request a Chlamydia self testing kit. This service offered is completely confidential and your information will not be shared with anyone. The test itself is really simple. All that you have to do is complete your sample (Urine / Swab) and post this back to us with your form in the box provided.
